On Thursday April 3, 2014, First Coast Service Options (First Coast), the Medicare Administrative Contractor (MAC) for Jurisdiction 9 (Florida, Puerto Rico, and the U.S. Virgin Islands), updated their website to acknowledge that they were having a processing issue in regards to G0461 and G0462, which are the new  immunohistochemistry codes for 2014.

The First Coast web update states that claims submitted with procedure codes G0461-26 and G0462-26 with units of service greater than one were denied in error for dates of service January 1, 2014, through March 25, 2014.  The web update also indicates that the issue was corrected on March 25, 2014 and that they (First Coast) will be automatically reprocessing all impacted claims. The expected date for reprocessing the impacted claims is May 3, 2014, so you will want to monitor your remittances to make sure any affected claims are reprocessed.
